Evidence shows that expert patients have improved wellness and better outcomes. Data from app users in Wales have shown a decrease in A&E visits and GP appointments, as well as less reliance on rescue medication to stay well.

An interactive 60-minute live virtual event for all primary care healthcare professionals who manage patients living with Chronic Disease in the Community, reflecting on these findings, how it will benefit the patients, practitioners, and the wider healthcare system.

This engagement event will be hosted on Microsoft Teams and the event link will be sent out on the morning of the event. The event will be recorded, and a video made available on the All Wales ICST Platform for those unable to attend the event live.
