Where ILD is suspected, spirometry and chest x-ray are recommended as initial investigations.

On spirometry, the typical pattern of abnormality in ILD is a restrictive pattern; reduced FVC, reduced FEV1, normal FEV1/FVC ratio.

There are many radiological patterns associated with ILD, but typically there is reticular shadowing at the lung bases.

If ILD is suspected following examination and initial investigations, refer to secondary care and request CT thorax at the same time as referral.
